ABSTRACT

Introducción: En la reconstrucción de los defectos cutáneos nasales se han utilizado varias técnicas a nivel mundial. Novedosas prácticas utilizan los concentrados plaquetarios por sus propiedades moduladoras y favorecedoras de la regeneración tisular. Objetivo: Determinar los resultados estéticos asociados al uso de la membrana de fibrina autóloga rica en plaquetas y leucocitos, para la regeneración de defectos cutáneos nasales posquirúrgicos. Métodos: Se realizó un estudio cuasiexperimental, longitudinal y prospectivo en el Servicio de Cirugía Maxilofacial del Hospital Universitario "Arnaldo Milián Castro" de la ciudad de Santa Clara, Villa Clara, Cuba, desde septiembre de 2015 hasta junio de 2019. Se efectuó un muestreo intencional, no probabilístico, de 31 pacientes. A partir de una donación de sangre del paciente, se obtuvo una membrana de fibrina rica en plaquetas y leucocitos, la que fue suturada en el defecto nasal posquirúrgico. Se estudiaron variables como el resultado estético, evaluado sobre la base de parámetros como la elasticidad, el color, el volumen y la superficie. Resultados: La edad media fue 62,71 ± 11,09 años y el 61,29 por ciento de la muestra correspondió al sexo masculino. Mostraron un buen resultado estético el 90,32 por ciento de los pacientes, lo que se asoció de forma significativa al grado de epitelización. Conclusiones: Se obtuvo un buen resultado estético en los pacientes donde se utilizó la membrana de fibrina autóloga rica en plaquetas y leucocitos, con independencia de la presencia de diabetes mellitus y hábitos tóxicos. El resultado estético estuvo asociado significativamente al grado de epitelización y a la edad(AU)


Introduction: In the reconstruction of nasal skin defects, several techniques have been used worldwide. Innovative practices use platelet concentrates for their modulating properties and favoring tissue regeneration. Objective: Determine the aesthetic results associated with the use of autologous fibrin rich in platelets and leukocytes, for the regeneration of post-surgical nasal skin defects. Methods: A quasi-experimental, longitudinal and prospective study was conducted in the Maxillofacial Surgery Service of "Arnaldo Milián Castro" University Hospital in the city of Santa Clara, Villa Clara province, Cuba, from September 2015 to June 2019. An intentional, non-probabilistic sampling of 31 patients was carried out. From a blood donation from the patient, a fibrin membrane rich in platelets and leukocytes was obtained, which was sutured in the post-surgical nasal defect. Variables such as the aesthetic result were studied, evaluated on the basis of parameters such as elasticity, color, volume and surface. Results: The mean age was 62.71 ± 11.09 years and 61.29 percent of the sample corresponded to the male sex. A good aesthetic result was shown by 90.32 percent of the patients, which was significantly associated with the degree of epithelialization. Conclusions: A good aesthetic result was obtained in patients where the fibrin membrane rich in leukocytes and autologous platelets was used, regardless of the presence of diabetes mellitus and toxic habits. The aesthetic result was significantly associated with the degree of epithelialization and age(AU)

RESUMEN Introducción: La reconstrucción nasal con colgajo frontal es frecuente en pacientes de edad avanzada, por lo general de causa oncológica. Con frecuencia se requieren otras técnicas complementarias, cuando los defectos comprometen varias subunidades nasales. Por lo que, reconstruir la nariz en pacientes jóvenes constituye siempre un gran desafío al cirujano plástico, por su repercusión social y la baja autoestima que afecta a estos pacientes. Objetivo: Mostrar las técnicas quirúrgicas empleadas en la reconstrucción nasal de un caso complejo, en el Instituto Nacional de Oncología y Radiobiología de Cuba. Caso clínico: Paciente masculino de 25 años de edad, con antecedentes de salud hasta sufrir un accidente por caída de una bicicleta, que ocasionó la amputación de la punta nasal y parte del ala nasal izquierda, con daño en los cartílagos alares. Conclusiones: La reconstrucción nasal con colgajo frontal y colgajo nasogeniano doblado sobre sí mismo, constituyen una adecuada herramienta quirúrgica del cirujano plástico para reconstruir defectos oncológicos y traumáticos(AU)


ABSTRACT Introduction: Nasal reconstruction with frontal flap is frequent in elderly patients, usually for oncological cause. Other complementary techniques are often required when the defects involve several nasal subunits. Therefore, nose reconstruction in young patients is always a great challenge for the plastic surgeon, due to its social repercussions and the low self-esteem affecting these patients. Objective: To show the surgical techniques used in the nasal reconstruction of a complex case, at the National Institute of Oncology and Radiobiology of Cuba. Clinical case: 25-year-old male patient, with a medical history until he suffered an accident due to a fall off a bicycle, which provoked the amputation of the nasal tip and part of the left nasal wing, with damage to the ala cartilages. Conclusions: Nasal reconstruction with frontal flap and nasolabial flap folded on itself constitute an adequate surgical tool for the plastic surgeon to reconstruct oncological and traumatic defects(AU)

Abstract Introduction: Packing of the nasal cavity has traditionally been used for postoperative bleeding control and decreasing synechia formation in patients undergoing nasal surgeries. Although absorbable nasal packing has been gaining popularity in the recent years, nonabsorbable nasal packing is still often used in nasal surgeries in various parts of the world. It is known to be associated with pain and discomfort especially upon and during removal, and previous reviews have only evaluated the effects of local anesthetic infiltration of nasal packing in septal surgeries. Objective: To evaluate the effect of infiltrating nasal packing with local anesthetics in postoperative pain and anxiety following sinonasal surgeries Materials and methods: We searched the PubMed and Embase databases from their earliest record to April 27, 2019, randomized controlled trials and prospective controlled trials for review, and included only randomized controlled trials for data analysis. We included studies using topical anesthetics-infiltrated nasal packing following sinonasal surgeries and evaluated the effectiveness compared to placebo packing in pain reduction during postoperative follow up, as well as the effectiveness in anxiety reduction. Results: Among 15 studies included for review, 9 studies involving 765 participants contributed to the meta-analysis. In terms of pain reduction, our analysis showed significant standard mean differences regarding effectiveness at postoperative 1, 12, 24 h interval for all surgical groups combined, in the sinus surgery group, as well as during nasal packing removal. There was no consistent evidence to support the effectiveness in anxiety reduction. Conclusions: Our study supports anesthetics infiltration of nasal packing as an effective method in managing pain in patients with nasal packing after sinonasal surgeries. However, the level of evidence is low. More high-quality randomized controlled trials are needed to establish its effectiveness in reducing anxiety. We believe this review is of great clinical significance due to the vast patient population undergoing sinonasal surgeries. Postoperative local hemorrhage remains the greatest concern for ear nose and throat surgeons due to the rich vasculature of the nose and sinuses. Sinonasal packing provides structural support and serves as an important measure for hemostasis and synechia formation. Although absorbable packing has been gaining popularity in the recent years, nonabsorable packing materials are still used in many countries due to lower cost. Infiltration of nasal packing with local anesthetic provides a solution to the discomfort, nasal pressure and nasal pain experienced commonly by the patients as evidenced by our analysis.


Resumo Introdução: O tamponamento da cavidade nasal tem sido usado tradicionalmente para controle do sangramento pós-operatório e diminuição da formação de sinéquia em pacientes submetidos a cirurgias nasais. Embora o tamponamento nasal absorvível tenha ganhado popularidade nos últimos anos, o tampão nasal não absorvível ainda é frequentemente usado em várias partes do mundo. Sabe-se que o tamponamento está associado a dor e desconforto, especialmente na sua remoção, e revisões anteriores avaliaram apenas os efeitos do tampão com anestésico local em cirurgias do septo nasal. Objetivo: Avaliar o efeito do tamponamento nasal infiltrado com anestésicos locais na dor e ansiedade pós-operatórias após cirurgias nasosinusais. Material e métodos: Para a revisão, pesquisamos nos bancos de dados PubMed e Embase desde o registro mais antigo até 27 de abril de 2019, incluímos ensaios clínicos controlados e randomizados, a ensaios clínicos prospectivos controlados e apenas ensaios clínicos controlados e randomizados para análise de dados. Incluímos estudos que usaram tamponamento nasal infiltrado com anestésicos tópicos após cirurgias nasosinusais e avaliamos a eficácia em comparação com o tamponamento com placebo na redução da dor durante o acompanhamento pós-operatório, bem como os efeitos na redução da ansiedade. Resultados: Entre os 15 estudos incluídos, 9, que envolveram 765 participantes, contribuíram para a metanálise. Em termos de redução da dor, nossa análise mostrou diferenças médias padrão significantes em relação à eficácia no pós-operatório nos intervalos de 1, 12, 24 horas para todos os grupos cirúrgicos combinados, no grupo da cirurgia sinusal e durante a remoção do tamponamento nasal. Não houve evidências consistentes para apoiar a eficácia na redução da ansiedade. Conclusões: Nosso estudo apoia o uso de tamponamentos nasais infiltrados com anestésicos locais como um método eficaz no tratamento da dor em pacientes após cirurgias nasosinusais. No entanto, o nível de evidência é baixo. São necessários mais ensaios clínicos randomizados de alta qualidade para estabelecer sua eficácia na redução da ansiedade. Acreditamos que esta revisão seja de grande significado clínico devido à vasta população submetida a cirurgias nasosinusais. A hemorragia local pós-operatória continua a ser a maior preocupação para os cirurgiões otorrinolaringológicos devido à rica vasculatura do nariz e seios nasais. O tamponamento nasosinusal fornece suporte estrutural e serve como uma medida importante para a hemostasia e formação de sinéquias. Embora o tamponamento absorvível tenha ganhado popularidade nos últimos anos, os materiais de tamponamento não absorvíveis ainda são usados em muitos países devido ao menor custo. A infiltração do tamponamento nasal com anestésicos locais fornece uma solução para desconforto, pressão e dor nasal comumente referida pelos pacientes, como evidenciado por nossa análise.

ABSTRACT

La atresia congénita de coanas se caracteriza por la presencia de placas que obliteran la comunicación entre la cavidad nasal y la nasofaringe desde el nacimiento. Se considera como incompatible con la vida cuando la condición es bilateral. El siguiente es el caso de una paciente femenina, de 17 años, sin síndromes asociados, con obstrucción y descarga nasal anterior bilateral, asociado a respiración oral desde el periodo neonatal. El diagnóstico de atresia bilateral de coanas fue confirmado por medio de endoscopía nasal y tomografía computarizada (TC). La imagen confirmó la presencia de placas atrésicas de composición mixta. La paciente recibió tratamiento quirúrgicamente por vía transnasal con resección de las placas y modelado de neocoana.


Congenital choanal atresia is characterized by the presence of plates obliterating the communication between the nasal cavity and the nasopharynx from birth. If bilateral, this condition is incompatible with life. This following is the case of a 17-year-old female patient, without associated syndromes, with bilateral nasal obstruction and anterior discharge, associated to oral breathing, starting in her neonatal period. The diagnosis of bilateral choanal atresia was confirmed by nasal endoscopy and computed tomography (CT) scan, due to the presence of atretic plates of mixed composition. The patient was surgically treated with plate resection and modeling of a neochoana by means of a transnasal surgical approach.

Abstract Introduction Chronic rhinosinusitis is a broad clinical syndrome characterized by mucosal inflammation of the nose and paranasal sinuses. In order for the paranasal sinuses to maintain their physiological functions; the ostiomeatal complex drainage pathways must be open. Surgical procedures are an important treatment option in patients who do not respond adequately to medical treatment. Although the methods and instruments used in functional endoscopic sinus surgery have continued to improve in recent years, the scar tissue formed during operation disrupts the drainage of the sinuses and reduces postoperative success. The natural ostiodilatation method, which is performed by balloon sinoplasty method, has become more and more popular in recent years. Objectives To compare the technique of balloon sinoplasty with the classical functional endoscopic sinus surgery method by considering the severity of chronic sinusitis on the same patient. Methods Total of 61 chronic sinusitis patients was included in the study. Paranasal sinus tomography of the patients was taken and according to the Lund-Mackay scoring, chronic sinusitis levels were determined. Cases were divided into two groups: Group 1 (severe chronic sinusitis group) and Group 2 (mild chronic sinusitis). Results There was no statistically significant difference in the results of comparison of sinuses which underwent balloon sinoplasty and classical functional endoscopic sinus surgery in Group 2 after Lund-Mackay scores. However in Group 1, the results of the comparison of postoperative Lund-Mackay scores of the balloon sinoplasty and the classical endoscopic operation were statistically significantly lower than those of the face half operated with the classical functional endoscopic sinus surgery. Conclusion The success of balloon sinoplasty in patients with mild sinusitis is the same as in classic functional endoscopic sinus surgery. However, as the severity of sinusitis increases, the efficacy of balloon sinoplasty decreases.


Resumo Introdução A rinossinusite crônica é uma síndrome clínica ampla, caracterizada por inflamação da mucosa nasal e seios paranasais. Para que os seios paranasais mantenham suas funções fisiológicas, as vias de drenagem do complexo ostiomeatal devem estar abertas. Os procedimentos cirúrgicos são uma importante opção de tratamento em pacientes que não respondem adequadamente ao tratamento clínico. Embora os métodos e instrumentos utilizados na cirurgia endoscópica funcional dos seios paranasais tenham melhorado continuamente, o tecido cicatricial formado durante a cirurgia interrompe a drenagem dos seios nasais e reduz o sucesso pós-operatório. O método natural de dilatação ostial, que é aplicado por meio da técnica de sinuplastia com balão, tem se tornado cada vez mais popular nos últimos anos. Objetivos Comparar a técnica de sinuplastia com balão com o método convencional de cirurgia endoscópica, no mesmo paciente, considerando a gravidade da rinossinusite crônica. Método Foram incluídos no estudo 61 pacientes com rinossinusite crônica. Por meio de tomografia dos seios paranasais e de acordo a escala de Lund-Mackay foram determinados os graus da rinossinusite crônica. Os casos foram divididos em dois grupos: Grupo 1 (grupo com rinossinusite crônica grave) e Grupo 2 (rinossinusite crônica leve). Resultados No Grupo 2 não houve diferença estatisticamente significante, segundo a escala de Lund-Mackay, no resultado da comparação dos seios paranasais nos quais foram realizadas a sinuplastia com balão e por cirurgia endoscópica convencional. No entanto, no Grupo 1, os resultados da comparação dos escores pós-operatórios de Lund-Mackay mostraram-se estatística e significativamente melhores naqueles submetidos à cirurgia endoscópica funcional dos seios paranasais. Conclusão O sucesso da sinuplastia com balão em pacientes com rinossinusite leve é o mesmo da cirurgia endoscópica funcional dos seios da face tradicional. No entanto, à medida que a gravidade da rinossinusite aumenta, a eficácia da sinuplastia com balão diminui.

Introduction: Chronic rhinosinusitis (CRS) is a highly prevalent pathology in our society. Due to the prevalence of this condition and to the persisting symptoms despite an appropriate medical treatment, surgical techniques are often required. Lately, minimal invasive techniques have been described, such as lacrimal diversion devices (LDDs). This technique offers a fast and convenient choice for delivery of sinus irrigation and topical medication. Objective: We aimed to describe our experience with LDDs and evaluate the safety and effectiveness of the procedure in patients with moderate to severe CRS without nasal polyposis (CRSsNP) and persistent symptomatology despite medical therapy. Methods: A total of 7 patients underwent bilateral lacrimal stents placement in the operating room. A retrospective observational study was conducted. The Sino-Nasal Outcome Test-20 (SNOT-20) survey was performed and the score obtained was compared before and 1 month after the procedure. Results: The LDDs were used for an average of 80 days. During the follow-up, only three patients had a mild complication with the device (granuloma in the punctum, obstruction, and early extrusion). The mean baseline SNOT-20 score dropped significantly ( p = 0.015) from 25.85 to 11.57 (mean: - 14.29) 1 month after the procedure. Conclusion: According to our experience and results, the use of LDD is a novel, feasible, and less invasive technique to treat refractory CRS. It reduces the risk of mucosal stripping, provides short-term outcomes, and the surgical procedure does not require advanced training in endoscopic sinus surgery. Moreover, it can be performed in-office under local anesthesia or sedation (AU)

Abstract Introduction: The most common cause of septoplasty failure is inferior turbinate hypertrophy that is not treated properly. Several techniques have been described to date: total or partial turbinectomy, submucosal resection (surgical or with a microdebrider), with turbinate outfracture being some of those. Objective: In this study, we compared the pre- and postoperative lower turbinate volumes using computed tomography in patients who had undergone septoplasty and compensatory lower turbinate turbinoplasty with those treated with outfracture and bipolar cauterization. Methods: This retrospective study enrolled 66 patients (37 men, 29 women) who were admitted to our otorhinolaryngology clinic between 2010 and 2017 because of nasal obstruction and who were operated on for nasal septum deviation. The patients who underwent turbinoplasty due to compensatory lower turbinate hypertrophy were the turbinoplasty group; Outfracture and bipolar cauterization were separated as the out fracture group. Compensatory lower turbinate volumes of all patients participating in the study (mean age 34.0 ± 12.4 years, range 17-61 years) were assessed by preoperative and postoperative 2 month coronal and axial plane paranasal computed tomography. Results: The transverse and longitudinal dimensions of the postoperative turbinoplasty group were significantly lower than those of the out-fracture group (p = 0.004). In both groups the lower turbinate volumes were significantly decreased (p = 0.002, p < 0.001 in order). The postoperative volume of the turbinate on the deviated side of the patients was significantly increased: tubinoplasty group (p = 0.033). Conclusion: Both turbinoplasty and outfracture are effective volume-reduction techniques. However, the turbinoplasty method results in more reduction of the lower turbinate volume than outfracture and bipolar cauterization.


Resumo Introdução: A causa mais comum de falha da septoplastia é a hipertrofia das conchas inferiores não tratada adequadamente. Diversas técnicas foram descritas até o momento: turbinectomia total ou parcial, ressecção da submucosa (cirúrgica ou com microdebridador) e a fratura lateral. Objetivo: Neste estudo, comparamos os volumes pré e pós-operatório da concha inferior com hipertrofia compensatória com o uso de tomografia computadorizada entre pacientes submetidos a septoplastia e turbinoplastia ou fratura lateral com cauterização bipolar. Método: Este estudo retrospectivo incluiu 66 pacientes (37 homens e 29 mulheres) internados em nosso serviço de otorrinolaringologia entre 2010 e 2017 por obstrução nasal e submetidos à cirurgia por desvio de septo nasal. Os pacientes submetidos à turbinoplastia devido à hiperplasia compensatória da concha inferior formaram o grupo turbinoplastia; aqueles submetidos à fratura lateral e cauterização bipolar foram separados, formaram o grupo fratura lateral. Os volumes compensatórios da concha inferior de todos os pacientes que participaram do estudo (idade média de 34,0 ± 12,4 anos, faixa de 17 a 61 anos) foram avaliados por tomografia computadorizada dos seios paranasais nos planos axial e coronal no pré-operatório e aos dois meses do pós-operatório. Resultados: As dimensões transversais e longitudinais do grupo turbinoplastia no pós-operatório foram significantemente menores do que as do grupo de fratura lateral (p = 0,004). Em ambos os grupos, os volumes da concha inferior diminuíram significantemente (p = 0,002, p < 0,001, respectivamente). O volume pós-operatório da concha do lado do desvio aumentou significantemente no grupo turbinoplastia (p = 0,033). Conclusão: Tanto a turbinoplastia como a fratura lateral são técnicas efetivas de redução de volume. No entanto, a turbinoplastia causa maior redução do volume da concha inferior do que a fratura lateral com cauterização bipolar.

RESUMEN Introducción: Los pacientes con lesiones cutáneas presentan cambios en su apariencia personal, no solo por la lesión propiamente dicha, sino también por los tratamientos quirúrgicos a que deben someterse. En la región facial estas lesiones son claramente evidentes y poseen mayor importancia estética. Objetivo: Caracterizar un caso de reconstrucción nasal alar tridimensional por exéresis de carcinoma basocelular en paciente geriátrico. Caso clínico: Paciente masculino de 81 años de edad con antecedentes patológicos personales de hipertensión arterial, que refiere presentar lesión nasal de 5 meses de evolución con crecimiento progresivo. Al examen físico facial se detecta alteración del contorno del ala nasal derecha por una lesión con impresión diagnóstica de carcinoma basocelular. Luego del chequeo preoperatorio se decide la exéresis tridimensional de esta, y reconstrucción mediante injerto cartilaginoso de concha auricular y colgajo pediculado nasogeniano. A las cuatro semanas se programa el segundo acto quirúrgico para la desconexión del colgajo pediculado. Conclusiones: El colgajo pediculado de la región nasogeniana combinado con injerto cartilaginoso autólogo constituye una excelente opción terapéutica que permite la reconstrucción nasal tridimensional, en la cual se obtienen buenos efectos estéticos y funcionales, combinados con una alta satisfacción del paciente por los resultados(AU)


ABSTRACT Introduction: Patients with skin lesions have changes in their personal appearance, not only because of the injury itself, but also because of the surgical treatments they must undergo. In the facial region these lesions are clearly evident and have greater aesthetic importance. Objective: Characterize a case of three-dimensional alar nasal reconstruction by exeresis of basal cell carcinoma in a geriatric patient. Clinical case: An 81-year-old male patient with a personal history of hypertension reports the presence of a nasal lesion of 5-month evolution with progressive growth. At facial physical examination an alteration is detected of the contour of the right nasal wing due to a lesion with a diagnostic impression of basal cell carcinoma. After the preoperative check-up, it is decided to perform three-dimensional exeresis and reconstruction by cartilaginous graft of the auricular concha and nasogenian pedicled flap. At four weeks, the second surgical act is scheduled for disconnection of the pedicled flap. Conclusions: The nasogenian pedicled flap combined with an autologous cartilage graft constituted an excellent therapeutic option that allowed three-dimensional nasal reconstruction, obtaining good aesthetic and functional effects, as well as high patient satisfaction with the results obtained(AU)

ABSTRACT

O lentigo maligno (LM) é uma forma de melanoma in situ que mais comumente se apresenta como uma mácula de crescimento lentamente progressivo, pigmentada, na face de idosos com pele danificada pelo sol. Esse melanoma in situ tem um risco (30% a 50%) de progressão para lentigo maligno melanoma. A excisão cirúrgica completa da lesão requer margens de pelo menos 10mm, mesmo para lesões in situ. Porém, quando o crescimento de LM ocorre em áreas de implicações estéticas ou funcionais (face, pescoço, solas), a excisão é frequentemente reduzida para preservar estruturas anatômicas importantes e por razões cosméticas. Além disso, as margens periféricas podem ser clinicamente mal definidas e nem sempre pigmentadas, com extensão subestimada e risco de ressecção insuficiente. A "técnica de espaguete", descrita por Gaudy Marqueste, é uma cirurgia estratégica baseada na amostragem de uma faixa de tecido "spaghettilike" para determinar as margens da lesão antes da remoção do tumor. Após a confirmação anatomopatológica de margens livres de neoplasia, a lesão principal central é ressecada, permitindo a reconstrução do defeito no mesmo procedimento, sendo uma alternativa à cirurgia micrográfica de Mohs.(AU)


Lentigo maligna (LM) is a melanoma in situ that commonly presents as a macula with progressive and irregularly pigmented growth, especially in the face of elderly people with sun-damaged skin. This melanoma in situ has a risk (30-50%) of progression to lentigo maligna melanoma. Complete surgical excision of the lesion requires margins of at least 10 mm, even for lesions in situ. However, when the growth of LM occurs in areas of aesthetic or functional implications (face, neck, and soles), the excision is often reduced to preserve important anatomic structures and for cosmetic purposes. Moreover, the peripheral margins may be clinically ill-defined and not always pigmented, and thus, such cases are associated with underestimated extension and risk of insufficient resection. The "spaghetti" technique, described by Gaudy Marqueste, is a strategic surgical approach based on sampling of a range of "spaghetti-like" strips to determine the margins of the lesion prior to removal of the tumor. After the pathological confirmation of neoplasia-free margins, the main central lesion is resected, allowing reconstruction of the defect in the same procedure, as an alternative to Mohs micrographic surgery. Introdução: A reconstrução nasal é a mais antiga das cirurgias plásticas. A anatomia nasal é complexa e necessita de uma associação de técnicas para a restauração da função e estética nasal adequada. Pereira et al. descreveram uma técnica que possibilita a reconstrução nasal total da cartilagem alar, com o uso de um enxerto da cartilagem auricular, com mínima deformidade auricular secundária à retirada do enxerto. O objetivo deste trabalho é apresentar uma modificação da técnica acima descrita, que possibilita reconstruir mais uma região anatômica do nariz, sem aumentar a morbidade, realizada por Collares et al., e a sua inserção no protocolo de reconstrução nasal total do Hospital de Clínicas de Porto Alegre. Métodos: Foi realizado um estudo retrospectivo. Avaliou-se a inserção da modificação da técnica em 10 pacientes que realizaram reconstrução nasal total. Resultados: Após a análise dos 10 casos, utilizando a modificação da técnica inserida no protocolo de reconstrução nasal total do Hospital de Clínicas de Porto Alegre, encontramos uma melhoria da forma do nariz, a válvula nasal interna com preservação da função e sem sequelas secundárias à retirada do enxerto auricular. Conclusão: Nesta série de casos, a modificação da técnica de Max Pereira resultou em tratamento estético funcional adequado quando implementada no protocolo de reconstrução nasal total do Hospital de Clínicas de Porto Alegre, sem aumentar a morbidade na área doadora.


Introduction: Nasal reconstruction is the oldest plastic surgery technique. The nasal anatomy is complex and requires an association of techniques for the restoration of function and adequate nasal esthetics. Pereira et al. described a technique that allows total nasal reconstruction of the alar cartilage through the use of an auricular cartilage graft, with minimal deformity secondary to the donor site. The objective of the present study is to present a modification, by Collares et al., of the technique described above, which allows the reconstruction of another anatomical region of the nose without increasing morbidity, and its insertion into the total nasal reconstruction protocol of Hospital de Clínicas of Porto Alegre. Methods: A retrospective study was conducted. We evaluated technique modification in 10 patients who underwent total nasal reconstructions. Results: After examining the 10 patients who were treated with the modified total nasal reconstruction protocol at the Hospital de Clínicas of Porto Alegre, we observed an improvement in the nose shape and internal nasal valve with preservation of function, without sequelae secondary to auricular graft removal. Conclusion: In this case series, the modification of the Max Pereira technique resulted in adequate aestheticfunctional treatment when implemented in the total nasal reconstruction protocol of the Hospital de Clínicas of Porto Alegre, without increasing the morbidity in the donor area.

INTRODUÇÃO: A busca por melhores alternativas para o tratamento do carcinoma basocelular (CBC) nasal é referida por diversos autores há anos. Os enxertos e os retalhos cutâneos apresentam problemas, como o risco de não integração ou de necroses; e ainda determinam a formação de cicatrizes que, muitas vezes, são extensas ou inestéticas. Além disso, eles demandam um tempo operatório maior, sendo realizados em centro cirúrgico. A exérese simples do CBC nasal, seguida da cicatrização por segunda intenção e de cuidados cosmiátricos simples no pós-operatório, ressurge como alternativa válida para o tratamento destes tumores. MÉTODOS: Os autores vêm apresentar sua experiência com a técnica, discutindo as vantagens do método. No período de 4 anos, todos os pacientes submetidos à técnica foram acompanhados prospectivamente. RESULTADOS: Dos 23 pacientes operados, 12 foram acompanhados por 2 anos e incluídos na avaliação. O procedimento cirúrgico teve duração média de 21 minutos (+6) e foi realizado sem dificuldades. A evolução pós-operatória se deu sem intercorrências ou recidivas tumorais. CONCLUSÃO: A técnica empregada coloca-se como uma alternativa simples, segura e com excelentes resultados para o tratamento dos CBC nasais.


INTRODUCTION: The search for better alternatives for the treatment of nasal basal cell carcinoma (BCC) has been reported by several authors. The complications associated with grafts and skin flaps include the risk of non-integration or necrosis, and the formation of scars that are often large or unaesthetic. In addition, these procedures require longer operative times, which implies the need for a surgical center. The simple removal of nasal BCC, followed by second intention healing and simple postoperative cosmetic care, has reemerged as a valid alternative for the treatment of these tumors. METHODS: In this study, the authors describe their experience with the technique, and report the advantages of the method. All patients submitted to this procedure were followed-up prospectively for a period of 4 years. RESULTS: Of 23 patients, 12 were monitored for 2 years and were included in the study. The surgical procedure was performed without difficulty in an average of 21 minutes (+6). Postoperative evolution was uncomplicated and without tumor recurrence. CONCLUSION: This technique is a simple and safe alternative, providing excellent results for the treatment of nasal BCC.

INTRODUÇÃO: A reconstrução nasal é desafiadora devido à posição central e proeminente do nariz na face. Pequenos defeitos nessa região tornam-se óbvios. Análise retrospectiva das 146 cirurgias de reconstrução nasal nos Serviços Integrados de Cirurgia Plástica do Hospital Ipiranga - SICPHI, avaliando a incidência dos defeitos nasais, assim como as principais técnicas cirúrgicas realizadas de acordo com cada região nasal. MÉTODOS: Estudo descritivo retrospectivo pela revisão de 109 prontuários e arquivos fotográficos, dos pacientes atendidos nos Serviços Integrados de Cirurgia Plástica do Hospital Ipiranga, em São Paulo, SP, no período compreendido entre janeiro de 2011 a junho de 2014. RESULTADOS: Foram realizadas 146 cirurgias. A faixa etária média foi de 67 anos. Em relação ao tipo de tumores malignos encontrados, 104 (73%) eram carcinoma basocelular e três (2%) carcinoma espinocelular. As 36 (25%) lesões restantes eram de características benignas. As regiões nasais mais acometidas foram: asa nasal, 55 (38%); dorso, 36 (25%); ponta, 26 (18%); lateral, 12 (8%); não especificada, sete (5%); comprometimento total do nariz, cinco (3%); e columela, dois (1%). Observou-se a preferência ao fechamento primário dos defeitos. No caso de impossibilidade do mesmo, realizaram-se retalhos locais, concordando com a literatura. CONCLUSÃO: As subunidades estéticas nasais podem ser reconstruídas por uma variabilidade de técnicas cirúrgicas. O cirurgião deve buscar um adequado resultado estético e funcional.


INTRODUCTION: Nasal reconstruction is challenging because of the central position and role of the nose in the face. Minor defects in this region become obvious. A retrospective analysis of 146 nasal reconstruction surgeries in the Integrated Plastic Surgery Services of the Ipiranga Hospital (SICP-HI) was performed, evaluating the incidence of nasal defects, as well as the main surgical techniques performed in accordance with each nasal region. METHODS: A retrospective descriptive study through a review of 109 medical records and photograph archives of patients treated in SICP-HI, São Paulo, SP, from January 2011 to June 2014. RESULTS: A total of 146 surgeries were performed. The mean patient age was 67 years. Regarding the type of malignant tumors found, 104 (73%) were basal cell carcinoma and 3 (2%) were squamous cell carcinoma. The remaining 36 (25%) lesions were benign. The most affected nasal regions were as follows: nasal wing, 55 (38%); the dorsum, 36 (25%); tip, 26 (18%); sidewalls, 12 (8%); unspecified, 7 (5%); total involvement of the nose, 5 (3%); and the columella, 2 (1%). There was a preference for primary closure of the defect. However, if this was infeasible, local flaps were used, in agreement with the literature. CONCLUSION: The nasal aesthetic subunits can be reconstructed using a variety of surgical techniques. The surgeon should seek an appropriate aesthetic and functional outcome.

INTRODUÇÃO: Reconstruções de defeitos complexos na face constituem um desafio para os cirurgiões. Após ressecção da lesão, o reparo nasal é difícil, já que possui uma variedade de tecidos e área local doadora reduzida para reconstrução. Já as lesões de lábio possuem como principal dificuldade a restauração da função, priorizando a integridade da estrutura esfincteriana, a continência oral, a abertura bucal e a sensibilidade. Entre as alternativas para reconstrução dessas áreas, podemos optar por retalhos locais, entre os quais o nasogeniano. OBJETIVO: Analisar desfechos de reconstrução nasal e de lábio superior em dois tempos cirúrgicos, utilizando retalho nasogeniano, após defeito complexo causado por ressecção de carcinoma espinocelular (CEC). RELATO DO CASO: Paciente do gênero feminino, 56 anos, apresentou CEC de progressão avançada em região nasal da columela, terço anterior do septo e parte do lábio superior. A paciente foi submetida à reconstrução com retalho nasogeniano e, em segundo momento cirúrgico, ao enxerto de cartilagem costal para reconstrução de columela. Apresenta evolução com boa integração e viabilidade dos enxertos e retalhos. O resultado estético foi satisfatório. CONCLUSÃO: A utilização de retalho nasogeniano como opção terapêutica para a reconstrução parcial de nariz e de lábio superior apresentou desfechos funcionais/estéticos favoráveis. Além disso, a segmentação do procedimento cirúrgico traz segurança na utilização dos retalhos locais, principalmente em indivíduos de difícil cicatrização.


INTRODUCTION: Reconstruction of complex facial defects is a challenge for surgeons. After the excision of a lesion, nasal reconstruction is challenging because of the variety of tissues involved and the reduced local donor area for reconstruction. On the other hand, a major difficulty in the reconstruction of lip lesions is restoration of function, with the priority being the maintenance of the integrity of the sphincter structure, oral continence, mouth opening, and sensitivity. Among the alternatives for the reconstruction of these areas are local flaps, including the nasolabial flap. AIM: To analyze the outcome of nasal and upper lip reconstruction performed in two surgical stages, by using a nasolabial flap for complex defects resulting from the resection of squamous cell carcinoma (SCC). CASE REPORT: A 56-year-old female patient presented with advanced SCC in the nasal region of the columella, anterior third of the septum, and part of the upper lip. She underwent reconstruction with a nasolabial flap and, in a second surgical procedure, with a costal cartilage graft for the reconstruction of the columella. The procedures resulted in good integration and viability of grafts and flaps. The aesthetic result was satisfactory. CONCLUSION: The use of a nasolabial flap as a therapeutic option for the partial reconstruction of the nose and upper lip produces favorable functional and aesthetic outcomes. In addition, the division of the surgical procedure into stages provides safety in the use of local flaps, particularly in patients with poor healing.

ABSTRACT

Os autores revelam, no presente trabalho, suas abordagens frente às alterações estéticas da columela nasal, mais observadas em sua prática clínica, a columela oculta e a columela pendente. Para a columela oculta os autores descrevem o uso de um retalho condro-mucopericondral do septo nasal como escolha preferencial. Para o tratamento da columela pendente os autores utilizam as abordagens, direta e indireta, descritas por Randall, nos casos leves a moderados, e para os casos mais acentuados utilizam uma associação de táticas além de uma modificação do que é descrito por Armstrong e Joseph. Suas opções de tratamento são confrontadas com o que há descrito na literatura da especialidade e seus resultados comparativos são apresentados.


This study discusses approaches for aesthetic correction of nasal columellas, i.e. retracted and hanging columellas, most commonly observed in clinical practice. We describe a chondral-mucoperichondrial nasal septal flap and direct or indirect approaches described by Randall as our preferred procedures to correct retracted and mild to moderate hanging columella, respectively. A combination of procedures is used in more severe cases, in addition to modified Armstrong and Joseph's approaches. These treatment options are compared to other literature reports on this specialty.

La septoplastía es uno de los procedimientos quirúrgicos más frecuentes en otorrinolaringología, cuya principal indicación es la presencia de desviación septal nasal significativa. Este enfoque quirúrgico ha evolucionado en forma considerable en las últimas décadas, siendo la septoplastía endoscópica la técnica más reciente. Esta técnica moderna, basada en la septoplastía convencional, ha mostrado beneficios en cuanto al procedimiento, resultados y complicaciones. El objetivo de este artículo es revisar las indicaciones, técnica, ventajas y principales resultados de la septoplastía endoscópica.


Septoplasty is one of the most common surgical procedures in otolaryngology, whose main indication is the presence of significant nasal septal deviation. This surgical approach has evolved considerably in recent decades, and endoscopic septoplasty is the most recent technique. This modern technique based on conventional septoplasty has shown several benefits in terms of procedure, results and complications. The aim of this article is to review the indications, techniques, advantages and main results of endoscopic septoplasty.

Background: Oronasal communication occurs after total maxillectomy for advanced sinonasal cancers. This results in feeding, breathing and cosmetic impairment. Various methods have been described to close off the palatal defect from the oral cavity to improve the function of speech and deglutition. Aims: The object of this article is to describe our experience of preservation of palatal mucoperiosteum for oronasal separation. Materials and Methods: Retrospective review of clinical and operative records of 31 total maxillectomy patients where oronasal separation was achieved by the conventional technique of applying a maxillary obturator. The postoperative complications arising from the use of maxillary obturator for oronasal communication after total maxillectomy in these 31 patients were analysed. To avoid the complications encountered in these 31 patients we preserved and used the ipsilateral palatal mucoperiosteum for oronasal separation. This new technique was applied in 12 patients. The results are presented and compared. Results : A total of 43 patients underwent total maxillectomy for advanced sinonasal tumors. In 31 patients the conventional maxillary obturator was used for oronasal separation. Among these patients, 30 had crustation of the maxillary cavity, nasal regurgitation and cheek skin retraction in 15 each, trismus in eight, infection of skin graft donor site in seven, cheek movement during respiration in five and ill-fitting prosthesis in three. In 12 patients palatal mucoperiosteum was preserved and used for oronasal separation. The complications encountered in oronasal separation by palatal prosthesis were avoided in the modified procedure. Conclusions: We found that oronasal separation by preservation of palatal mucoperiosteum following total maxillectomy allowed excellent palatal function, prompt rehabilitation and minimal complications without compromising the prognosis.

Most studies show that objective measures to quantify and determine surgical success in the treatment of nasal obstruction do not correlate with subjective improvement as reported by patients. AIM: To evaluate the subjective improvement of nasal symptoms in patients undergoing septoplasty with or without turbinectomy. MATERIALS AND METHODS: A prospective study. We evaluated 72 septoplasty patients with or without partial inferior turbinectomy; the patients answered a questionnaire preoperatively and on the 60th day after surgery. RESULTS: Septoplasty was done associated with bilateral partial inferior turbinectomy in 83.3 percent of patients; it was unilateral in 9.7 percent; there was no need for turbinate reduction in 6.9 percent. An improvement of all symptoms was observed after surgery. Nasal obstruction had improved in 68 patients (94.4 percent) by the 60th postoperative day. The average nasal obstruction score in patients with and without allergic symptoms was similar before surgery and on the 60th postoperative day. Older patients had milder preoperative allergic symptoms. CONCLUSIONS: Nasal symptoms in patients undergoing septoplasty, with or without turbinectomy, improved. Patients with and without allergic symptoms showed a similar improvement of nasal obstruction on the 60th postoperative day.


A maioria dos estudos mostra que medidas objetivas para quantificar e determinar o sucesso das cirurgias para a obstrução nasal não têm correlação com a melhora subjetiva relatada pelo paciente. OBJETIVO: Avaliar a melhora dos sintomas nasais nos pacientes submetidos à septoplastia, com ou sem turbinectomia. MATERIAL E MÉTODO: Estudo prospectivo. Foram avaliados 72 pacientes submetidos à septoplastia, com ou sem turbinectomia inferior parcial, que responderam ao questionário no préoperatório até o 60º dia de pós-operatório (PO). RESULTADOS: Foi realizada septoplastia associada à turbinectomia inferior parcial bilateral em 83,3 por cento dos pacientes, unilateral em 9,7 por cento e não houve necessidade de redução das conchas nasais em 6,9 por cento. Foi observada melhora para todos os sintomas no pós-operatório. Sessenta e oito (94,4 por cento) pacientes apresentaram melhora da obstrução nasal no 60º PO. A média da nota da obstrução nasal entre os pacientes com e sem sintomas alérgicos foi semelhante no pré-operatório e no 60º PO. Os pacientes mais velhos apresentaram menor intensidade dos sintomas alérgicos no pré-operatório. CONCLUSÕES: Os pacientes submetidos à septoplastia, com ou sem turbinectomia, evoluem com melhora de todos os sintomas nasais. Os pacientes com e sem sintomas alérgicos apresentam melhora da obstrução nasal de forma semelhante no 60º PO.

ABSTRACT

La rinoplastia abierta es un procedimiento quirúrgico que se utiliza para resolver los problemas estéticos o funcionales de la nariz mediante una incisión columelar externa, que se extiende hacia el vestíbulo nasal y que permite elevar la cubierta cutánea y dejar expuesto todo el esqueleto osteocartilaginoso. El empleo de este abordaje ha sido muy polémico desde que se hicieron las primeras publicaciones sobre su uso, fundamentalmente a causa de la cicatriz columelar que produce. No obstante, es un proceder aceptado por la mayoría de los cirujanos que se dedican a la cirugía nasal, sobre todo en casos complejos donde tiene obvias ventajas. Se describen los resultados obtenidos con esta técnica quirúrgica en un gran número de casos, atendidos en el Servicio de Cirugía Plástica del Hospital Hermanos Ameijeiras, desde la introducción de ésta en 1997 hasta julio de 2005. Se detallan la operación y sus indicaciones, las deformidades nasales más frecuentes y las ventajas obtenidas en cada grupo de pacientes. Se destaca la ausencia de complicaciones directamente relacionadas con la vía de abordaje y la excelente calidad de la cicatriz columelar resultante


The open rinoplastia is a surgical procedure that is used to solve the aesthetic or functional problems of the nose by means of an incision external columelar that extends toward the nasal lobby and that it allows to elevate the cutaneous cover and to leave exposed the whole skeleton osteocartilaginoso. The employment of this boarding has been very polemic since the first publications were made on its use, fundamentally because of the scar columelar that takes place. Nevertheless, it is a to proceed accepted mainly by most of the surgeons that are devoted to the nasal surgery, in complex cases where he/she has obvious advantages. The results are described obtained with this surgical technique in a great number of cases, assisted in the Service of Plastic Surgery of the Hospital Siblings Ameijeiras, from the introduction of this in 1997 until July of 2005. The operation and their indications, the most frequent nasal deformities and the advantages are detailed obtained in each group of patient. He/she stands out the absence of complications directly related with the boarding road and the excellent quality of the scar resulting columelar(AU)
